With Sam spiraling faster into repeating his mistakes and killing someone again, Dr. Strauss grows more impatient with his kidnapper patient. But as a prisoner and therapist, Alan proposes a new plan to Sam—family therapy with his mother. But even with a new motivation backing him, Sam’s urges seem to get the best of him.
1. Episode 4 Synopsis/Speculation
On the threshold of crisis, Sam honors the promise Dr. Strauss made him make as a condition for continuing their work together. The trust built between patient and doctor is tested, with a life in the balance.
Things turn dire in Episode 3 when Alan’s therapy isn’t helping Sam enough to stop him from killing again. Episode 4 will see Sam’s homicidal tendencies getting worse and we might get more of the Fortner family’s backstory. Alan will continue to devise a way from keeping Sam from deciding that his next victim is Dr. Strauss.

5. Episode 3 Recap
Alan meets Candace, Sam’s supposedly dead mother. He urges her to free him since nothing about this imprisoned therapizing is ethical or effective. But Candace remains adamant about keeping Dr. Strauss prisoner if it means helping Sam.
In the next session, Alan brings up Candace and Sam’s abusive father and how Sam felt once he left. Alan shared his idea of family therapy with Sam, to which the patient agreed.
Alan urges Sam to think of Candace, to stop himself from committing another murder, thinking it would protect his mother. Sam agrees, but in the following session confesses he put Alan’s latest idea to the test and found himself still imagining every detail of how he would kill his latest marked victim.
Alan believes since Sam still managed to abstain from killing, the family bond might work after all. But later that night, to Strauss’ horror, Sam drags a tied-up victim and shuts him behind a closed door.
